As a general rule you can only upgrade an item of equipment to the level of its most advanced counterpart. You can only upgrade helms to the highest level. Weapons can be upgraded to the equivalent level of a sword. However, this doesn't boost the weapon's damage output and could decrease its value in trade.

Requirements
